The Process
Fill out the form here so that we know you are interested in referring a patient.
Once we receive the email, you receive a call back the same week, confirming receipt of the email and seeking patient information over the phone in a HIPAA compliant manner. Please let us know during the call if you would like the patient to be referred back to your practice for follow up care.
Next, we contact the patient and schedule a visit with them.
During our patient intake process, we receive further information, such as relevant physicians’ and their contact information and latest medical and dental records. We complete all of this information prior so that during the first visit we are able to start, and hopefully, complete all necessary treatment. We can also prepare fresh dental records at the first visit as needed. Patient expectations of single visit dentistry are set at this time.
After completion of treatment, we send you a short message, providing you with an update.
Interdisciplinary Care Model

We routinely call physicians’ offices and speak to neurologists, endocrinologists, surgeons and physicians about our patients. Many of our patients are on blood thinners, diabetic medications and are on hospice receiving palliative care. Some of our patients require bedside dental treatments. A few patients are on 24×7 oxygen support.
We have a few patients with Special Health Care Needs and routinely use treatment modifications to meet their expectations. We come up with creative approaches and believe in meeting the patient where they are at. Sometimes that may mean focusing on small achievable goals. Other times it may mean offering single visit comprehensive dentistry with sedation.
We believe healthcare needs to be interdisciplinary. This means all healthcare providers- nurses, physicians, dentists, occupational therapists and others work together to help the patient get better.
